New Patients
We always welcome new patients, and we look forward to working with you should you decide to become part of our practice family.
Print and complete the appropriate form below before you come in for your visit.
Before Your First Visit
Before your first visit, we ask that you complete your intake forms at home and either send them to us in advance or bring them with you. This lets us focus on your health needs for the whole time you’ll be in our office.
You can download the forms here.
The more information you can give us about your dental/medical history and current needs and desires, the more easily we can tailor treatment to your unique dental situation.
At Your First Visit
The first time we see you, Dr. Karen will conduct a thorough oral health exam to document the current state of your oral health and devise a sensible plan for dealing with any oral health issues you may be experiencing. There will be plenty of time for you to ask questions and share your concerns. We want you to take an active role in your care, to work with us as partners in supporting optimal oral/systemic health.
Unless you can provide us with a high quality full mouth series of x-rays less than a year old, we will need to take a set at your first appointment. Simply, x-rays show us your teeth, gums, and bone with a depth impossible to achieve through just a visual exam. We take only digital images, which keeps your radiation exposure to a minimum.
Let Us Know of Any Sensitivities You Have
We normally use an essential oil diffuser utilizing several citrus oils, but we do remove this for our patients with sensitivities. If you wish to check how you might react to our diffuser, you’re welcome to stop by our office before your appointment.
Should you have other sensory needs, just let us know. Our goal is for you to feel as comfortable as possible during your time with us.
